Clients will agree to the following terms and conditions before engaging in any services offered by Erica Madden.

 

• Erica Madden is a Certified Nervous System & Somatics Coach. Coaching is not therapeutic counseling. If you have mental health concerns, they will need to be addressed by a licensed mental health care professional.

• Erica Madden provides coaching and services to offer confidential client-centered instruction. In sessions and other services, you will receive information, resources, and tools to help you balance your nervous system and enhance your well-being. You must be 18 or older to access services.

​

• Erica Madden does not diagnose, treat, cure, or prevent any diseases, disorders, or conditions. She is not a licensed medical doctor, nutritionist, or dietitian. Erica is a health care professional and licensed as a Registered Nurse. She may utilize nursing practice skills such as therapeutic communication and education within her scope while working in the capacity of a Nervous System & Somatics coach. However, she is not operating as a RN in this role.

• Each client's well-being is of the utmost importance to Erica. As appropriate, she may recommend a client work with a Medical or Mental Health Professional, or obtain screening for the suspected existence of diseases, disorders or conditions. This does not constitute medical advice or diagnosis and is not a substitute for medical consultation or treatment.

• No promises have been made or will be made as to the results of coaching or any services provided by Erica Madden.

​

• Client documents maintained by Erica Madden are considered private and confidential information. Erica places a high value on confidential information and will make every effort to ensure privacy (the video system used is HIPAA secure.) There are, however, some exceptions and limitations to confidentiality as required by law. These are: (1) Any known or reasonably suspected cases of child abuse or neglect. (2) Any known or suspected intentions of harming oneself (suicide). (3) Any known or suspected intentions of harming others. (4) Any situation when written consent is given by the client to release information. Erica Madden is a mandatory reporter.

• The use of technology is not always secure. Erica will make every effort to ensure privacy, and clients accept the risks of confidentiality loss in the use of email, phone, and other technology.

​

• Erica is here to support client goals. Clients agree to assume full responsibility for their own health and wellbeing, and to assume risk for any manner of loss, injury, claim or damage whatsoever, known or unknown, incurred as a result of working with Erica Madden.

​

This agreement is a full release of liability.
